Services and Fees

Dr. Siobhan Key Inc. (the “Provider”) shall provide a three-month group weight management coaching program for $3,000.00 USD (the “Coaching Program”) or 3 monthly instalments of $1,050 USD. The fees are inclusive of GST for Canadian clients. The Provider strives to use a holistic approach to address the physical, psychological and social cases of obesity and over-eating tailored for physicians. The Coaching Program includes access to the online course portal, instructional videos and workbook and a minimum of one group coaching call per week.

 

Group Coaching Calls

The Coaching Program includes access to weekly group coaching calls (the “Coaching Calls”). The Coaching Calls will be scheduled at the sole discretion of the Provider and may not fit your individual schedule.

 

The Coaching Calls will be recorded, and the recordings will be posted in the online course portal for all members of the group to access for the duration of the Coaching Program. By signing this agreement, you are consenting to the recording of the Coaching Calls for the purpose of sharing with all members of the group in the online course portal.

 

You are encouraged to participate fully in the Coaching Calls and agree to be respectful of other members of the group at all times.

 

Private Coaching Package

At a cost of $1,000.00 USD, inclusive of GST for Canadian clients, you may purchase an additional private coaching package which includes four (4) twenty-five (25) minute one-on-one virtual coaching sessions (the “One-on-One Coaching Calls”). The One-on-One Coaching Calls will be coached by certified life coaches and physicians (the “Coaches”). The Coaches will offer a range of availability for the One-on-One Coaching Calls. Booking of sessions will be on a first come first served basis and the Provider makes no guarantees about any specific availability for One-on-One Coaching Calls. All One-on-One Coaching Calls must be used within the 90 day Coaching Program. If you cancel a One-on-One Coaching Call on less than 24 hours-notice you will forfeit the call. Cancellations on more than 24 hours-notice may be rescheduled directly with the Coaches at a mutually convenient time.

 

Consent to Use of Electronic Communications

The Coaching Calls and One-on-One Coaching Calls will be conducted via videoconferencing technology (“Electronic Communication”). While the Provider will use reasonable means to protect the security and confidentiality of information sent and received by Electronic Communication, there are risks and the Provider cannot guarantee the security and confidentiality of Electronic Communication. Use of Electronic Communication can increase the risk of information being disclosed to third parties.

 

Confidentiality

All information shared in group coaching is confidential except in circumstances where a group member or someone else is in imminent danger or where disclosure is otherwise required by law. By signing this agreement, you are agreeing to keep the confidentiality of other participants of the Coaching Program.

 

Reproduction of Sharing of Content

You acknowledge that you have an individual license to access the Coaching Program and that any unauthorized sharing, reproduction or repurposing, including via social media platforms, of the content provided in the online course portal and during the Coaching Calls is strictly prohibited.

 

Limitation of Liability

Weight loss coaching does not create a doctor-patient relationship and should not be used as a substitute for medical care. You are responsible for your own actions and physical, mental and emotional well-being. You agree that the Provider is not liable for any of your actions or inactions. The Provider makes no representations or warranties and does not guarantee your results.

 

60 Day Risk Free Promise

If after completing a minimum of 50% of the Coaching Program material, you determine that the Coaching Program is not the right fit for you, the Provider will allow you to cancel your participation in the Coaching Program and receive your full $2,500.00 USD money-back. All requests for a refundable cancellation must be made in writing to the Provider within 60 days of commencement of the Coaching Program and you must have completed at least 50% of the coursework available on the online course portal to take advantage of this offer. The One-on-One Coaching Calls package is non-refundable.

 

Deferral Requests

The Provider understands that in a busy physician’s life, sometimes things come up. If during the first 60 days of the Coaching Program, you find you are unable to participate in the Coaching Program, you may send in a written request to the Provider for a one-time deferral to the next available Stress Eating SOS session. The Provider expressly reserves the right to deny requests for deferrals at their sole discretion and does not have to provide a reason for said decision.

 

Termination by the Provider

The Provider may, at any time, for any reason, terminate this Agreement.

 

Consent

I have read and understand the information in this form, the information in the Terms and Conditions, and the information in Appendix A. I voluntarily agree to participate in the coaching process.
